As a State Hospital Liaison, you will be responsible for supporting CHPW members who are transitioning to and from State Hospitals, Long Term Civilly Committed (LTCC) behavioral health beds, and other inpatient psychiatric facilities. This role involves facilitating intake, evaluation, and referral services for members in behavioral health settings. You will also help manage the State Hospital and LTCC populations to ensure alignment with the target census set by the Washington State Health Care Authority and the Behavioral Health Administration. CHPW is grateful to have the opportunity to celebrate Tacoma Area Coalition of Individuals with Disabilities TACID and its inclusive approach through a 2024 Advancing Equity Fund grant. TACID promotes the wellness, recovery and resilience of adults experiencing disabilities in Pierce County. This unique organization is peer-led and directed with all direct services provided by Certified Peer Support Specialists (CPCs) who have lived experience of disability or a behavioral health challenge. The $10,000 grant provides unrestricted funds that TACID can use in whatever way best meets their organization's needs. On October 14, 2024, our CHPW team traveled to Phoenix for CrisisCon24, where crisis partners, advocates, and payors came together for three days of learning, connecting, and growing. CHPW Sr. Director of Regional Systems Integration Connie Mom-Chhing and Regional Manager Marci Bloomquist were in attendance to learn more about updates on the National 988 Crisis Line and to hear from other states on how they are navigating their crisis services. Services represented at this conference included crisis call centers, mobile crisis teams, crisis residential programs, peer support and respite programs, psychiatric urgent care, and crisis stabilization programs. On Saturday, November 2, CHPW/CHNW CEO Leanne Berge and Sea Mar Community Health Centers Medical Director Dr. Shane Dunaway presented their shared view that voting is good for your health, as part of an opinion piece in The Columbian. Here's an excerpt from "Dunaway and Berge: Voting is good for your health": Voting is good for one’s health. In fact, the American Medical Association declared in 2022 that “voting is a social determinant of health,” meaning it’s a non-medical factor that affects a person’s health outcomes, just as other economic, environmental, and social conditions do. With the election just days away, it’s incumbent upon each of us to promote this civic engagement, particularly among groups of people whose right to vote has been historically disenfranchised, and many of whom concomitantly experience worse health outcomes. In health care, we see daily how social determinants like housing insecurity, poor education and exposure to environmental conditions affect patients’ health, and there is a growing body of evidence that shows communities that vote experience better health. A Health & Democracy Index looks at 12 indicators of public health and shows that states with more inclusive voting policies and higher levels of civic engagement are healthier. One theory for this correlation is that those who feel disempowered politically also feel disempowered in other aspects of their lives, which directly ties to poorer health outcomes. Washington’s network of community health centers, including Sea Mar Community Health Centers in Clark County, is mobilizing to support people in exercising this basic right of our democracy. Many are partnering with the national nonprofit Vot-ER in nonpartisan activities such as voter registration and election participation for their patients and health care staff. Casting our ballots is an act of patriotism. As not-for-profit organizations dedicated to serving historically underserved and marginalized communities through Medicaid, Medicare and the Health Benefit Exchange’s public option, we believe it is especially appropriate to facilitate and advocate for all eligible people to vote in their best interest, as a matter of health. There are diverse policy perspectives across the political spectrum that support justice, economic opportunity and better lives for our communities. Community Health Plan of Washington (CHPW) welcomes Dr. Aparna Gadekar, CHPW's Sr. Medical Director of Medicare, to the team. Dr. Gadekar reports to Chief Medical Officer/Chief Health Officer Demetria Malloy MD MSHS and will play a vital role in designing, establishing, and maintaining population health-based programs, and providing Medicare-focused guidance for CHPW’s utilization, pharmacy, and case management activities. She works with clinical leadership to improve the Medicare Care Model, particularly emphasizing the Dual-Eligible Special Needs Plan (D-SNP) Models of Care. Additionally, she works closely with Community Health Centers (CHCs) to enhance provider engagement and performance in quality metrics and risk adjustment. APARNA GADEKAR, MD, CHCQM, AMDCP is a board-certified internist with extensive experience as a hospitalist. Known for her compassion, she is deeply committed to providing timely and high-quality care to our most vulnerable populations. Her areas of expertise include Value-Based Care, Medicare Advantage, Star Ratings, data analytics, and strategic development. Dr. Gadekar earned her medical degree from Seth G.S. Medical College in Mumbai, India, affiliated with K.E.M. Hospital, where she received a scholarship for her studies. She completed her Internal Medicine Residency at Sinai Hospital of Baltimore, affiliated with Johns Hopkins University. In 2019, she was recognized as one of the Top 100 Physicians in Greater Grand Rapids by Grand Rapids magazine, a testament to her dedication and contributions to the medical field. Dr. Gadekar holds a Certificate in Healthcare Quality and Management (CHCOM) and has completed an Accelerated Management Development Certificate Program at the Michigan Ross School of Business Executive Education, as well as an online certification course in Healthcare Economics from Harvard University.
