A human error during a website upgrade caused the serious Post Office data breach earlier this year, which exposed personal details of hundreds of former subpostmasters, the public inquiry has been told. In June, a document containing the names and addresses of victims of the Post Office scandal was accidentally published on the Post Office website. It contained the names and addresses of all 555 subpostmasters who took part in the 2018/19 High Court Group Litigation Order (GLO) that proved bugs in the Post Office’s Horizon IT system were responsible for accounting losses for which the victims had been blamed and prosecuted. #postoffice #databreach

Change Healthcare says it has notified approximately 100 million Americans that their personal, financial and healthcare records may have been stolen in a February 2024 ransomware attack that caused the largest ever known data breach of protected health information. A notification letter from Change Healthcare said the breach involved the theft of: -Health Data: Medical records, doctors, diagnoses, medicines, test results, images, care and treatment; -Billing Records: Records including payment cards, financial and banking records; -Personal Data: Social Security number; driver’s license or state ID number; -Insurance Data: Health plans/policies, insurance companies, member/group ID numbers, and Medicaid-Medicare-government payor ID numbers. Change’s parent firm United Health Group had incurred $1.521 billion in direct breach response costs, and $2.457 billion in total cyber attack impacts. #ransomware #databreach

Data broker National Public Data has filed for bankruptcy, claiming it cannot sustain the mounting financial and reputational damage associated with a major data breach it suffered in December 2023. The background-checking service filed for bankruptcy, and explicitly cited the 2023 data breach as a direct contributor to its downfall.
