BioAge is pleased to announce plans to initiate a Phase 2 trial of our apelin receptor agonist azelaprag co-administered with tirzepatide for treatment of obesity. Under the terms of an agreement signed yesterday, Eli Lilly and Company will provide tirzepatide for the study, and Lilly’s Chorus organization will advise and assist BioAge on trial design and execution. The trial, planned to start in mid-2024, is designed to assess whether this drug combination can achieve increased overall weight loss with improved body composition.

• Azelaprag is an oral drug for the future of obesity therapy: Co-administration has the potential to achieve best-in-class weight loss with next-generation oral incretins currently in the pipeline, whose performance lags behind injectables. Our goal is 20–25% healthy weight loss with a completely oral drug combination.

• We are collaborating with a global leader in obesity: We are thrilled to work directly with the clinical development experts at Chorus and benefit from Lilly’s experience in obesity drug development. We are proud to collaborate with Lilly in one of their most important and competitive areas.

We got 2024 off to a great start with BioAge’s annual panel discussion & reception during JPM Healthcare Week. In a conversation moderated by BioAge CEO Kristen Fortney, panelists Tracey McLaughlin (Professor, Stanford University School of Medicine), Rob Myers, MD (CMO, OrsoBio), Michael Davidson (CEO, NewAmsterdam Pharma), and Axel Haupt (Senior Vice President, Diabetes Clinical Development, Eli Lilly and Company) shared their thoughts on our theme of “Extending human healthspan: Obesity, metabolism, and aging”.

Key takeaways from their conversation:

✅ Many pathways link obesity, metabolic disorders, and aging — and targeting these common pathways may have benefits for treating chronic disease.

✅ Emerging drug targets like apelin, FGF21, CETP inhibitors, and others have effects on aging pathways as well as specific diseases like obesity, diabetes, and CVD. Combination therapies may have synergistic benefits.

✅ Challenges in studying drugs for prevention include long trial durations, the need for large sample sizes, identification of appropriate biomarkers, and lack of reimbursement incentives. Regulators and stakeholders need to collaborate on solutions.

✅ Biomarkers are advancing: lipids and CRP have potential for tracking metabolic health; advanced imaging may eventually be accepted for showing benefits on atherosclerosis; and Alzheimer's disease trials are moving towards biomarker-based approvals.

✅ Key upcoming readouts will provide more evidence on whether incretins and new mechanisms can improve cardiovascular and kidney outcomes. Studies on preserving muscle mass with weight loss and combination approaches for obesity are important.

✅ Precision medicine approaches, targeting treatments based on disease subtypes and underlying physiology, will come to the fore as the cardiometabolic space attracts more attention and resources.

At abcam's 2023 Innate Sensing & Signaling in Immunology meeting in Boston this week, BioAge scientists Kevin Wilhelmsen, Sarah Tronnes, and Aditi Deshpande presented a poster on our novel class of NLRP3 inflammasome inhibitors, which we are developing for inflammatory and age-related diseases. 

BioAge’s AI-driven discovery platform revealed that NLRP3 levels rise with age and are positively correlated with all-cause mortality. To identify novel NLRP3 inhibitors, we screened billions of molecules in a DNA-encoded library (DEL) for compounds that bind purified NLRP3 protein, yielding drug-like compounds that functionally inhibit the NLRP3 inflammasome in vitro with comparable or superior potency relative to publicly known inhibitors.
