Critically ill patients on ventilators face a high risk of lower respiratory tract infections, such as ventilator-associated tracheobronchitis (VAT) and ventilator-associated pneumonia (VAP). VAP is linked to increased morbidity and mortality in the ICU, emphasizing the need for prevention. 

This review examines the potential of pre-emptive aerosolized antibiotics (AA) in preventing ventilator-associated infections and whether aerosolized treatment for VAT can halt its progression to VAP.

While there are chances to enhance antibiotic prescribing throughout the care spectrum, the discharge phase from hospitalization is a notable contributor to antibiotic overuse. 

Thus, it becomes imperative to improve antibiotic prescribing by optimizing antibiotic selection, reassessing antibiotic treatment, and using the shortest effective duration of therapy post-discharge.
