As New York State is preparing to implement the 1115 Medicaid Waiver, Social Care Networks (SCNs) are required to engage with and screen millions of Medicaid members for social health needs to improve access to social drivers of health interventions across the state.

Blooming Health is partnering with SCNs across New York and plans to support implementation of the 1115 Waiver to empower both SCNs and their CBO network with infrastructures for better engagement, screening, and connecting members to essential resources like nutrition, housing, and transportation.

We are currently utilized in over 33 New York counties and serving over 114,000 New Yorkers, and are well positioned to support this rollout.
