As a Transitions of Care Nurse at Thyme Care, you will play a critical role in ensuring a seamless transition through the oncology care continuum for our members. We are looking for a Case Manager skilled in admissions, discharges, transfers, and referral services to provide care for our members. Ideally, you are CCM or OCN certified and have 3+ years of case management experience.
