The ACO Reach program was born out of the idea that if we want to improve patient outcomes, we have to work smarter, not harder. Yet, even within the ACO Reach population, heart failure is still woefully undertreated, and the effect on patient outcomes and cost of care has been drastic. The difference between a patient being treated for heart failure* and a patient left unseen is a 50% reduction in mortality and thousands of dollars per month. ACOs with higher than average readmission rates see the cost-gap widen to $20,000 per patient over just the first three months after their HF diagnosis. For the 20 worst offenders, the cost grows further, sitting between $30,000-$110,000; for ACOs of any size, this inaction is costly and lethal.  

The science is out: we know GDMT works, we know it saves money, and most importantly we know it saves lives. Yet still, undertreatment is endemic to our system. Across the entire ACO reach program, we’ve found there is a staggering $650,000,000 difference between the treated and untreated in their first 3 months of care, post-HF diagnosis.

Finerenone (Kerendia) - a nonsteroidal MRA that already has great CV and renal data in patients with DM2 - meets its endpoint for a significant reduction in the combined endpoint for mortality, HF hospitalizations and acute care visits for worsening HF in patients with HFpEF. Excited to see full data soon!

The treatment of HFpEF just got more interesting! 

Topline results from the SUMMIT trial released today showed that obese patients with HFpEF treated with tirzepatide (Mounjaro) - a GLP-1RA - lost 15.7% of their body weight  compared to 2.2% for patients in the usual care group. Patients treated with tirzepatide also had fewer HF events and greater improvements in symptoms and quality of life scores versus those treated with placebo. 

Patients in the tirzepatide arm were also more likely to require less diuretics than those in the control arm. 

Great to see more meaningful therapeutic options for our patients with HFpEF!

Hopefully approvals and indications will expand to make these agents easier to get for all of our HF patients who meet criteria.

Why are we diagnosing so much HF in the ER??

More that 1 million new cases of HF are diagnosed each year in the US. Early recognition of HF can lead to more rapid initiation of GDMT and save lives. 

In Circulation:HF, researchers at Stanford analyzed data from nearly 1,000,000 HF patients who were newly diagnosed with HF in an Optum database spanning 2003-2019. 

Of the 959,438 patients diagnosed with new HF in the database, 38% (!) of these were diagnosed in acute care settings (predominantly the ER). Nearly half (46%) of the cases had potential HF symptoms documented in their chart over the previous 6 months. 

Women and Black patients were more likely than men and White patients to be diagnosed in an acute setting versus as an outpatient. 

With our latest HF trials showing a statistically significant benefit in outcomes within just 12 days of medication initiation (EMPEROR-Reduced), we need to find ways to identify and treat HF earlier particularly in patients with early symptomatology. 

Biomarkers, screening home echos and newer low EF detection devices are all tools we now have in our arsenal to make this possible.

Lack of cardiology clinic access and strained resources can be overcome with digital/virtual HF partnerships.
