Lymphedema is both a challenging condition to treat and a major annoyance for anyone who has it. The condition causes swelling primarily in one of your arms or legs - sometimes in both - but may also affect other areas of your body. This swelling happens because your lymphatic system, an important part of the immune system, isn't working properly or is damaged as a result of surgery (especially for breast cancer), radiation treatment, infections, or injuries. Or, some people are born with a lymphatic system that doesn’t work properly. The buildup of lymph fluid exerts pressure on the surrounding tissues, leading to pain and discomfort. The pain can be constant or intermittent and may range from a dull ache to a more intense throbbing sensation. The discomfort can be exacerbated by prolonged periods of standing or sitting, physical activity, or tight clothing. Sodium is an essential mineral and electrolyte that helps regulate the balance of fluids in our bodies, maintains proper nerve function, and supports muscle activity. Our bodies require a small amount of sodium to function, but too much of it can increase the risk of heart failure, kidney disease, stroke, and heart attacks. Yet, nearly 90% of Americans consume more than the daily maximum of 2,300 milligrams (mg) recommended by the Food and Drug Administration (FDA)—equivalent to one teaspoon of salt. On average, U.S. adults consume about 3,400 mg of sodium per day. "For best heart health, we really recommend 1,500 mg daily. So, the average American is eating more than double the optimal amount of sodium per day," says Nate Wood, MD, MHS, an internal medicine, obesity medicine, and primary care physician with special expertise in culinary medicine. An estimated 20% of people in the United States have a severe form of acid reflux called gastroesophageal reflux disease (GERD), in which the backflow is more frequent and, over time, can damage the esophagus. People with GERD may rely on a proton pump inhibitor (PPI), a type of medication that lowers stomach acid production; common ones include omeprazole (Prilosec®), lansoprazole (Prevacid®), and esomeprazole (Nexium®). Although some can take PPIs for as little as two weeks to feel better, those with serious forms of GERD may need to be on PPIs indefinitely. PPIs are generally considered safe, but concerns about long-term use have been raised over the years—especially as they have been widely used and sometimes, overused, doctors say. Nobody likes getting a needle. But epinephrine auto-injectors, such as EpiPens®, have long been the primary treatment for anaphylaxis, a serious allergic reaction some people have in response to triggers such as peanut butter, eggs, and other foods; bee and other insect stings; and antibiotics, aspirin, and other medications. Without immediate treatment, these reactions can progress rapidly and be potentially life-threatening within a matter of minutes. But now there is neffy®, a new emergency nasal spray. It’s the first epinephrine product for anaphylaxis treatment that doesn’t involve a needle. The Food and Drug Administration (FDA) approved neffy in August 2024 for adults and children who weigh at least 66 pounds after giving the product a “Fast Track” designation, which expedites the review of drugs that fill an unmet need for people with serious conditions. Anaphylaxis can affect the whole body, cause such complications as respiratory collapse, and be fatal without quick medical treatment and a visit to the emergency room. “Many people are very excited about having this new option for patients,” says Mario Rodenas, MD, a Yale Medicine allergist and immunologist. “But, as with anything else, we have to be cautious with neffy,” he says. A fear of needles and logistical hurdles are just two reasons some people may skip their annual flu vaccine. Recently, the Food and Drug Administration (FDA) removed those obstacles by approving the at-home use of FluMist®, an influenza vaccine administered by nasal spray. “Making this vaccine available to use at home is a great step for public health purposes,” says Scott Roberts, MD, a Yale Medicine infectious diseases specialist. “There are always patients who don’t get vaccinated because they can't get to the clinic, or it's not top of mind. If you can offer this at home via self-administration, I imagine it's only going to increase the percentage of people who are vaccinated against the flu. Plus, it’s the only needle-free option.
