Oracle Health recently announced its new AI-driven EHR platform, developed independently of Cerner’s original system and designed to transform how clinicians interact with health records. With features like voice commands and an “AI-first” approach, the platform aims to remove traditional menus and streamline clinician workflows. Key Highlights: - Voice-Activated Interface: Clinicians can access records with simple questions like, “Please pull up a patient’s latest lab results.” - AI-Integrated Core: Unlike many EHRs adding AI as a secondary feature, Oracle ’s system embeds AI from the start, adapting to user behavior over time to enhance efficiency. - Transition Support: Current Cerner Corporation users may face migration decisions, but Oracle Health promises a smooth transition backed by their advanced database for seamless data handling. I was asked by someone at a health tech company today what some of the unsolved problems in healthcare are. Here are a few that plagued me when I practiced primary care in a rural area of Kansas (some of these also apply to urban practices): 1. Lack of interoperability among disparate systems. As a primary care physician (PCP), I wished my EHR (which was Meditech at the time) would interact with those from nursing homes, assisted living facilities, home health companies, and hospital systems. I needed to exchange records with or send orders to these systems. In a rural area, for example, my orders and messages to nursing homes and home health companies were via hand-written faxes. 2. Lack of access to various specialists and types of care. For example, I was at least unaware of any local medical resources for my patients that had substance abuse problems such as alcohol or drug addiction. Patients would travel 2 to 4 hours (depending on where they went) to seek inpatient rehab. We also did not have access to Endocrinology, Infectious Disease, Gastroenterology, Rheumatology (other than one physician whose panel was full in a nearby town), Neurology, and several subspecialist surgical services such as Neurosurgery, Vascular Surgery, and others. Patients in rural areas often refuse to drive to see specialists. This made figuring out how to help them difficult at times. 3. Large amounts of clerical work. I think I received over 100 EHR inbasket tasks per day and about 30-40 faxed tasks per day to deal with. I had about 1600 patients. I saw about 15-20 patients per day when working 8-5. It was difficult to keep up. I got burned partly since about 2/3 of my time was spent on clerical work compared with 1/3 of the time seeing patients. Unfortunately, I didn't know if I was helping anyone either because the only quality data the health system would give me were patient satisfaction scores. 4. Medical errors/medication adherence/medication reconciliation. We didn't have robust clinical decision support (CDS) in our EHR. I hope CDS continues to improve using AI or other means, especially in smaller health system/small practice EHRs. We also were a rural hospital, so I didn't have a pharmacist that worked directly with my clinic that can sometimes help with some of these issues in urban areas. I hope some tech companies out there start to focus on some of the neglected health systems out there, such as those in rural areas where there are fewer resources.

It’s commonly said that automation has eliminated exactly one job since 1950—elevator operator. But there may soon be a second—medical scribe. Scribes emerged in the early 2010s as the ultimate workaround for clunky EHRs. Rather than deal with navigating the EHR or satisfying onerous documentation requirements, some physicians started employing young adults (primarily pre-medical students) to sit in on their visits and write notes, enter orders, and perform other functions. The scribe industry boomed, with the largest vendor (ScribeAmerica) alone employing 26,000 scribes. However, human scribes are expensive ($15-$25/hour), require training, and frequently turnover. AI scribes now threaten their role. AI (ambient) documentation has emerged as the leading GenAI use case in healthcare. A land grab is happening before our eyes, with companies like Abridge, Nabla, Nuance, Suki, Deep Scribe, Ambience, and many others fiercely competing for new customers. AI scribes never tire, do not leave to go to medical school, do not require prolonged training, and, although far from free, are much less expensive than their human counterparts. Many AI scribes create surprisingly good notes, even though physicians are technically supposed to review them for errors and the notes may not precisely match their preferred style. There is also some regulatory uncertainty. Still, the tech is rapidly progressing. For now, human scribes have the edge on related activities (such as entering codes, orders, etc.), but many AI scribe companies are quickly moving to cover those activities, too. There will always be tasks that AI scribes cannot do – such as chaperoning physical exams, offering physicians companionship, or even getting coffee. Still, the number of human scribes will significantly wane, and they may eventually join elevator operators as one of the only jobs that automation has eliminated. https://lnkd.in/gXFMVAQE
