The big culprits include sleep disordered breathing (apnea), insomnia, and sleep deprivation. All of these reduce your brain's ability to restore itself and ward off chronic disease. In this talk I will focus on how digital solutions like SleepSpace can help with issues regarding falling asleep and staying asleep, in addition to improving sleep quality. We will discuss the recent Blinded Placebo Controlled Randomized Trial that our team completed through funding from the National Institute Of Aging where we showed that when a trained psychologist uses this technology to enhance the recommended treatment for insomnia, it is safe and can make the intervention more effective. Dr. Dan Gartenberg here, sharing my personal saga of tackling sleep apnea. Last time I posted on this, I was diving into my sleep apnea evaluation using this cool new device called Wesper . It lets you check your breathing health right from your home and connects you with an MD for a diagnosis. So, I went all in with Wesper.co , tracking my sleep over three days, and got my official diagnosis of mild to moderate sleep apnea from a connected MD. Armed with this diagnosis, I headed to my dentist and got the ball rolling with my insurance to cover a mandibular advancement device from Panthera. It’s like a mouth guard that pushes your lower jaw forward, which is backed by hundreds of studies for treating mild to moderate sleep apnea. Insurance only covered half since I have an EPO, but with a PPO, you'd get around 80-90% covered. It usually costs about $3k out of pocket… If you are a dentist, chiro, or sleep specialist, please reach out to me at dan@sleepspace.com about how you can incorporate this into your practice. After a couple of weeks, my dentist crafted my MAD device. It's super simple to put in and use, and once in place, it pushes my lower jaw forward, opening up my airway. The next step was crucial—ensuring this device was actually working. For severe cases, CPAP is usually the go-to, but there are other treatments like Inspire, or you can consult an ENT for more options. But since my case was mild to moderate, I opted to try the MAD first. I used Wesper again to validate the device's effectiveness. Initially, my moderate apnea showed an apnea-hypoxia index of 13.2 on the more conservative metric (3% oxygen desaturation) and 7.8 on the less conservative metric (4% oxygen desaturation). Ideally, you want fewer than five events per hour to avoid a sleep apnea diagnosis, but aiming for one or two per hour would be optimal for health. After retesting with the device, my apnea events dropped significantly. The more conservative metric showed 5.5 events per hour, and the less conservative metric showed just 2.1 events per hour. This improvement indicated that the device was effectively treating my sleep apnea. Looking ahead, I'm planning to incorporate weight loss into my treatment and then retest without the device to see the impact. The results are in on our team's Randomized Controlled Trial (RTC) that explores a new way to augment conventional Cognitive Behavioral Therapy for Insomnia (CBTi) administered by a therapist. In this study, we demonstrate improved outcomes when a therapist uses the system we developed on key sleep and adherence metrics. The work was accepted at the American Academy of Sleep Medicine and the Sleep Research Society annual conference (APSS) and was distinguished by being selected for a presentation. The presentation will be given by our amazing collaborator Margeaux Gray . If you are attending the conference, please check it out and join the conversation! Here's what we did. We created a novel IoT intervention designed to increase adherence to CBTi directives. For example, lights and sounds adjusted in people's homes in order to increase adherence to a prescribed bedtime and wake time. Meditations and relevant digital content were also made available on the platform - in addition to the consensus sleep diary. Data was collected from nearable and wearable devices (apple watch) and could be shared with a therapist. This helped hold participant's accountable for their behaviors. Aspects of this system are currently available in the SleepSpace platform. The full publication is still pending, with additional results on blood-based biomarkers and actigraphy. By monitoring these patterns, you can identify issues and optimization tricks. For example, track naps and alcohol use to determine how these factors impact your sleep. One way that I use the dashboard is by tracking my average bedtime over weeks, months, or years to see what sleep schedule works best to improve my sleep quality. In addition to tracking your own sleep on the web dashboard, you can request to view the sleep of a loved one or someone who you are coaching on their sleep health. This can help that person be held accountable for their sleep choices, which is proven to then improve their sleep quality. To truly understand sleep, it is important to track both your perceptions of sleep and your sleep quality evaluated by wearable and nearables. This is why the web dashboard includes a toggle that you can switch to either view sleep diary data or wearable/nearable data. We then do the work of calculating your most relevant sleep statistics.
