Congratulations to the great team at J.P. Morgan for hosting an exceptional healthcare conference last week.
 
It was a fantastic opportunity to network and learn from so many #healthcareinnovators and explore ways to collaboratively promote greater #healthequity and access. We also appreciated the opportunity to share some of the meaningful outcomes Somatus is driving in the #valuebasedcare space for patients living with #chronickidneydisease and associated chronic conditions, including:
•	31% reduction in hospitalizations for patients participating in the Somatus program
•	17% improvement in cost of care for patients participating in the Somatus program
•	4.1x higher adoption of disease-delaying medication in African American program participants
 
All of these improvements are instrumental in helping us to deliver More Lives, Better Lived!
